- Leads a multidisciplinary team of healthcare providers to achieve excellence in health care
- Collaborates to address quality, safety and risk management issues, including the development of improvement plans to address shortfalls. Collects and assesses key performance indicators for the programs and initiates quality improvement/process changes as required
- Leads program development and design of clinical workflows that support new models of care delivery for the target populations, in collaboration with the operations leadership team and system partners
- Participates in reporting key performance indicators and program metrics, to evaluate the efficacy and sustainability of programs
- Manages the medical equipment and supplies required for hospital-sector programs, including ordering/returning to the appropriate vendor and ensuring equipment/supplies are used within the program budget
- Leads or assists with the integration of client satisfaction initiatives; ensures that all referred client concerns and complaints are satisfactorily addressed and documented in a timely manner
- Develops educational initiatives and supports the ongoing development of the field staff working with health system partnership programs. Supportive of developing best practice Champions within the collaborative multidisciplinary team
Qualifications
- Current CNO certificate as Registered Nurse, degree preferred.
- Minimum of 5 years of recent experience with progressive responsibilities as a Nursing Manager, preferably in a community setting
- Experience managing in a union environment is an asset
- Solid knowledge of the principles, practices, and methods of community nursing and service delivery to patients as well as program development, implementation and evaluation
- Valid G driver’s license and reliable vehicle
