Southlake is excited to announce new opportunities in our innovative Extensivist Clinic, opening in early 2025. A first-of-its-kind, intensive outpatient care program designed to deliver leading-edge, patient-centered care. This Clinic is a collaboration of a dedicated inter-disciplinary team, including Nurse Practitioners, RNs, RPNs, Social Workers, Pharmacists, and others, to ensure comprehensive patient care.
The Extensivist Clinic will provide integrated care for patients to prevent unnecessary admissions and ensure safe, earlier discharges with coordinated follow-up. It provides a broader approach to sustaining patient health and stability over time.
Building on lessons learned from the successful Southlake@home program which has now spread across the province, this Clinic serves as a critical bridge between inpatient and outpatient care and community services. It offers stabilization and ongoing follow-up for patients Located on our hospital campus in Newmarket, the Clinic takes advantage of Southlake’s robust resources and expertise to enhance patient outcomes.

Be a part of our exciting future, apply today!JOB SUMMARY:The Advance Practice Nurse/Acute Care Nurse Practitioner will:
- Engage in direct and in-direct advanced clinical practice, education, research and consultation within the area of internal medicine to ensure the highest quality of care is provided to patients and families.
- Actively participate as part of the program and professional leadership team in promoting quality care by implementing evidence based practices, evaluating patient outcomes, development and evaluation of policies, clinical programs and services.
- Model both the professional and organization core competencies and provide expert clinical nursing leadership.
- Provide comprehensive specialty care focusing on health promotion, disease prevention, and chronic disease management. Working in collaboration with the team, the NP offers patient-centered care, including assessment, diagnosis, and treatment, while adhering to the top of their scope of practice.
Primary Responsibilities
- Regularly track and evaluate patients’ symptoms and health status to identify early warning signs of deterioration. Take proactive steps to prevent hospitalizations and worsening of conditions.
- Conduct routine and comprehensive health evaluations for patients with chronic illnesses, monitoring key health indicators to proactively manage their conditions.
- Promote adherence to prescribed medication regimens by educating patients on the significance of compliance, potential side effects, and proper actions to take in case of missed doses
- Serve as the first point of contact for the designated patient population, providing rapid access and response for urgent concerns and coordinating timely interventions, particularly for high-risk or complex chronic patients.
- Identify and diagnose new health conditions, including common illnesses and injuries, within the scope of routine primary care. Ensure timely and effective treatment interventions.
- Collaborate with the Interprofessional Care Team (IPCT) to develop, implement, and adjust individualized care plans based on patient progress and emerging needs.
- Provide ongoing education and support to patients and their families, empowering them with knowledge and self-management tools for chronic conditions.
- Coordinate referrals and facilitate communication with specialists, ensuring that their recommendations are integrated into the patient’s overall care plan for continuous and comprehensive care.
- Maintain an up-to-date understanding of available external programs and resources. Appropriately refer patients to these programs to enhance their care and provide additional support beyond the clinic setting.
- Facilitate smooth transitions for patients moving between different levels of care, such as from hospital to home or from primary care to specialist services, ensuring continuity and comprehensive management of their health and well-being.
- Work closely with the team to ensure seamless patient care by maintaining accurate documentation of all patient interactions and care plans in the electronic medical record (EMR) system.
Skills & Abilities
- Minimum 5 years of experience in a community-based medicine or primary care setting, hospital medicine ward, or equivalent. Experience working within a multidisciplinary team is preferred.
- Demonstrated strong communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholders
- Strong capabilities in clinical assessment and diagnostic processes.
- Comprehensive understanding of internal medicine practices, integrated care models, and effective multidisciplinary team collaboration.
- Awareness of and sensitivity to the diverse cultural, social, and economic backgrounds of Ontario’s patient population to ensure equitable and accessible healthcare.
- Dedication to continuous learning and staying up to date with current best practices.
- Ability to prioritize tasks, manage time efficiently, and adapt to a dynamic work environment.
- Proven experience in community and/or primary care settings.
- Possesses highly developed assessment, analytical, problem solving and critical thinking skills
- Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology
- Demonstrated excellent organizational and coordination skills and the ability to prioritize multiple competing workload demands
- Demonstrated ability to provide leadership, support and critical analysis in stressful, complex clinical and emotional situations
- Demonstrated commitment to customer service

- Bachelor of Science in Nursing (BScN) from an accredited institution is required, with a Masters in Nursing/Nurse Practitioner Stream, or Masters in Nursing/Master of Science in Nursing with successful completion of a post Masters Acute Care Nurse Practitioner Certificate program from a recognized school of graduate studies.
- Active registration in good standing with the College of Nurses of Ontario (CNO) as a Nurse Practitioner (RN(EC)).
- Current membership with the Registered Nurses’ Association of Ontario (RNAO) or the Nurse Practitioners’ Association of Ontario (NPAO), along with proof of liability insurance.
