Are you able to provide comprehensive support to clients, aiding them in achieving sustainable independence and stable housing? Join our Housing Plus Team!The Housing Outreach Worker (HOW) is part of an Intensive Case Management Team that will provide community-based support to participants connected to a Housing First Program. Reporting to the Housing Plus Manager, the Housing Outreach Worker provides client-centered care, landlord support and connection to mainstream services. They will be responsible for meeting one-on-one with clients on a regular basis to assist with goal setting, accessing supports and enhancing life skills to help them move towards sustainable independence. This will require a broad spectrum of supports that will vary from participant to participant, the Housing Outreach Worker will have a strong focus on client support related to locating and supporting participants towards appropriate tenancy.Employment Type: Full-time, Permanent
Schedule: Monday to Friday: 8:00 a.m. to 4:00 p.m. (may include some evenings and weekends)

RFR #: 958MAJOR DUTIES AND RESPONSIBILITIES

  • Maintain an active caseload, meet with clients on an on – going basis through in-person visits.
  • Provide written and verbal reports and assessments as requested.
  • Accompany Housing First participants to viewings and appointments at potential housing locations.
  • Support and advocate for participants through exploring informed choices regarding potential housing options.
  • Facilitate transition to the Follow Up Support Team either within agency or with partner agencies.
  • Maintain a strong relationship with landlords and maintain a working knowledge of relevant community resources and supports.
  • Provide crisis intervention and safety planning when necessary.
  • Attend all team meetings, work collaboratively with the larger housing team and external client supports.
  • Maintain client confidentiality and clear boundaries.
  • Maintain relevant, timely and accurate documentation, including data entry into ETO.
  • Participate in all staff training (e.g. First Aid, SafeTALK, Naloxone, Indigenous Awareness) and attend monthly general meetings and support meetings with manager.
  • Maintain professionalism and adhere to all policies and procedures including but not limited to Health & Safety and HR policies and procedures.

MINIMUM QUALIFICATIONS AND SKILLS

  • Undergraduate degree in Social Work or related field is preferred.
  • Two years’ experience working with clients at risk; direct experience with Housing First initiatives and previous case management considered a strong asset.
  • Understanding of barriers related to mental health, addiction, trauma and poverty.
  • Understanding of Housing First and Harm Reduction principles.
  • Able to maintain healthy boundaries, communicate care, and provide professional referrals.
  • Excellent interpersonal, communication and documentations skills.
  • Well-developed skills in relation to conflict resolution, advocacy and creative problem solving.
  • A self-starter with the ability to work with minimal supervision.
  • Valid class 5 driver’s license, personal vehicle and $2 million liability insurance are required.

WORKING CONDITIONS

  • This position is dependent on government funding and maybe adjusted as required.
  • Office work environment as well as visitation in the community with clients, travel out of office is required.
  • Physical effort is minimal, able to lift 50 lbs. routinely. Must be able to walk upstairs, to accompany community participants to public buildings and private homes.
  • This position requires varied mental and emotional effort, must be able to respond appropriately to intense situations, provide conflict resolution, and work directly with clients at risk.
  • Employees are to be aware that many of our clients may experience difficulties or impairment at times, which could result in them uttering threats, discriminatory. statements, and other insults at your person. This may in a raised voice, or in seeing unpredictable behaviour up to and including the possibility of them becoming aggressive.
  • Training on how to respond is provided, but this is to be considered typical working conditions. Many of our clients are unaware of the impact of these comments.

  • This position requires the use of a personal vehicle for company use.
  • Must have legal authorization to work in Canada on a full-time basis for anyone other than current employer.
  • Must have a valid driver’s license and maintain $2M Vehicle Liability insurance on the vehicle being used.
  • A clear Driver’s Abstract is required prior to offer of employment.
  • Vulnerable Sector Check and Employment Security Check is required.
