Job Description: SalaryThe salary range for this position is CAD $42.56/Hr. – CAD $52.95/Hr.SummaryWithin the context of a patient, client and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff.Reporting to the Clinical Coordinator, Foundry Virtual, and receiving overall direction from the Professional Practice Leader, Occupational Therapy, the position supports the interdisciplinary Virtual Service team through the Foundry Work & Education program. Provides occupational therapy services to enable clients referred to the Foundry Virtual Services Team to achieve and maintain optimal health outcomes by helping achieve independent living, education and vocational goals. Performs assessments on client’s occupational performance utilizing an evidence-based Individualized Placement Support (IPS) approach for employment; plans, develops and implements outcome oriented psychosocial rehabilitation interventions for clients; evaluates the effectiveness of the services; prepares and maintains records; participates in team meetings, quality improvement initiatives, and interdisciplinary research; and demonstrates safe and effective resource utilization and quality management. Collaborates with family members/significant others, program partners, inter-disciplinary team members, educational resources and other health care professionals/providers to achieve excellence in health services, education and evidence-based evaluation. Supports program development for Foundry Virtual BC employment services including developing employment groups, materials and guides for clients, and resources to support providers.What You BringEducation
Master’s Degree in Occupational Therapy and three (3) years’ recent related experience working with youth and young adults facing mental health and substance use issues in a community setting or an equivalent combination of education, training and experience. Current full registration as an Occupational Therapist with the College of Health and Care Professionals of BC (CHCPBC).Skills and Abilities

Acts as a clinical resource in meetings including network wide, Community of Practice meetings: provides expertise, input, clinical services, and recommendations to teams across the Foundry network with the goal of enhancing patient care, service delivery, and clinical outcomes in the Foundry Works program.Leads and supports program development for Foundry Virtual employment services including development of employment groups, materials and guides for clients as well as resources to support clinicians with assessment, motivational interviewing, etc.Supports a small case load of clients receiving vocational or educational supports through Foundry WorksProvides clinical supervision to and evaluation of students as required. Provides clinical direction to peer support worker as appropriate.Evaluates occupational therapy services, including quality and outcomes, in collaboration with the client and family in keeping with the client’s goals by determining the purpose, method and criteria of the evaluation, implementing the evaluation, and analyzing and interpreting results to utilize results for service delivery and future planning of occupational therapy services.Collaborates with interdisciplinary staff, health care professionals, school staff, clients/families/caregivers and other community resources in the identification and resolution of client/family/caregiver care issues, revision or termination of services, cultural and language barriers, ethical dilemmas, and coordination and integration of care, and makes referrals to other services providers, as appropriate.Maintains a variety of records including client files by gathering information, entering data into computerized databases and/or documenting information into approved formats according to the College of Occupational Therapists of BC practice standards. Notes client’s progress, changes to treatment plan, and records test/treatment results to provide evidence and rationale that support assessment findings and intervention plan. Compiles statistics on workload activities and summarizing information into written reports as requested.Participates in team/program activities required to enhance and optimize the delivery of care and to facilitate evidence-based practice. Promotes quality management principles for team/program operations, identifies processes/procedures that require improvement, and drafts/revises guidelines, policies and procedures for review.Participates on committees as directed to promote optimal support for clients, to facilitate the resolution of issues, to represent the rehabilitation goals on regional committees, and to advocate for the goals and objectives of the team/program.Participates in approved occupational therapy and interdisciplinary research and special projects in collaboration with designated personnel/team and other health care professionals, through methods such as collecting data, summarizing information, and/or providing verbal updates for review and further analysis by principal researcher, research team and/or project coordinator.Maintains and updates own clinical knowledge within area of practice, and develops a plan in collaboration with designated personnel/team for professional development. Reviews progress to ensure that goals are achieved within established time frames.Provides input into the clinical and operational budget needs of the program.Performs other related duties as required.
