The Supportive Housing Program provides housing for seniors who have been street involved, experiencing mental health and/or addictions issues. The program provides onsite psychosocial, primary care supports and services including: intensive case management; harm reduction and addiction supports; therapeutic and supportive counseling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up supportWhat You Will Do

  • Responsible for implementing all activities and meeting all qualitative and quantitative outcomes per the annual work plan developed with management within the stipulated timeframe.
  • Work with other members of the Community Connections team to determine mentorship needs, encourage the development of mentorship opportunities, and coordinate mentor retention strategies.
  • This position ensures that service is provided to newcomer clients in person and through e-services and that all program goals, statistics and outcomes are met.
  • Assist with outreach to the business community, professional associations, and other places to attract potential mentors.
  • Receive and review mentee applications, conduct in-depth interviews to determine current situation, goals and potential benefits from mentoring.
  • Collaborate with diverse employers to organize career sessions, providing newcomers with sector-specific opportunities and networking platforms.
  • Conduct in-depth interviews with mentors to determine key strengths (skills, experience, networking contacts, languages, cultural understanding) that are of potential benefit to mentees.
  • Develop and implement a process for matching mentees with mentors, including protocols and policies, leading to successful mentorship relationships and confirm matching arrangements, including mentoring objectives, period, frequency and method of meeting, location of one-on-one meetings to ensure safety and comfort of both parties; ensure both parties understand and sign the mentoring agreement.

What You Bring to the Team

  • Bachelor’s degree in Social Work, Community Work, Adult Education or a related field.
  • Minimum two (2) year’s’ experience and proven track record in supporting newcomers to achieve goals in counselling, employment, or related environments.
  • Experience in career coaching is a fundamental aspect of the role.

  • Experience providing guidance and support to clients seeking employment in various fields.
  • Previous experience in the non-profit social services sector is preferred.
  • Understanding of and sensitivity to issues facing immigrants, immigrant communities, and internationally trained professionals and extensive knowledge of community-based orientation and support resources, activities and events within the GTA area.
  • Proven experience in facilitating group workshops on diverse topics, focusing on supporting newcomers in community integration and delivering engaging orientation and training sessions tailored to the needs of diverse audiences.

What You Will Do

  • Plan, build, organize & facilitate onsite activities with staff and external partners.
  • Engage with tenants to complete an interest assessment of what types of programming tenants would be interested in partaking in.
  • Assess, develop, and build healthy and engaging program spaces.
  • Facilitate skill-building, healthy living and fun workshops for participants.
  • Create a calendar of planned monthly programming while liaising with external parties and staff with the development and co/facilitation of daily/weekly workshops & activities.
  • Create a participant intake pathway & marketing material to inform tenants/create excitement.
  • Create & implement monthly/quarterly Pre-& Post-program surveys.
  • Interact with individuals in person; providing emotional support, and feedback to case managers/counsellors and encouraging participation in programs.
  • Work with the case manager to carry out goals and expectations of the care plan and complete daily needs assessments to determine appropriate programming for participants.
  • Monitor clients, identify additional needs and mitigate risks.
  • Identify participants who may be interested in becoming peer helpers.
  • Collaborate with the community and build partnerships to bring in new programming.
  • Work collaboratively with community partners including mental health, substance use, geriatric crisis and mental health and justice, in order to maximize support of client and help create pathways to recovery goals.
  • Ensure that anti-oppression, community development, and harm reduction principles are achieved in the program.
  • Take part in team and unit meetings.
  • Participate in ongoing training and education.
  • Participate in regular supervisory sessions and annual performance appraisals.
  • Help to create a healthy and motivating work environment.

What You Bring to the Team

  • Bachelor’s degree or Social Service Worker with 5+ years’ experience creating and facilitating programming.
  • 2+ years of experience working with adults/seniors and homeless individuals with mental health/substance use issues.

  • Ability to lead, develop and facilitate programming.
  • Strong assessment and interpersonal skills; excellent verbal and written communications skills.
  • Ability to develop, implement and evaluate client/tenant needs.
  • Information and referral skills; ability to effectively link client to supportive services.
  • Self-directed and independent, also experienced in working effectively as a member of team.
  • Ability to minimize barriers through appropriately designed and managed service delivery.
  • Ability to work from a client-centered approach.
  • Knowledge of the health sector and its related services, and community resources.
  • Ability to follow policies, procedures, and adhere to legislation.
  • Well grounded; good boundary setting skills.
  • Understanding of clinical practices and care models.
  • Exercise good judgment, flexibility, and contribute to the effective functioning of a clients.
  • Strong leadership skills.
  • Ability to facilitate presentations and workshops.
  • Applied computer skills (MS word, excel, and Internet).
  • Ability to use case management software is an asset (Pirouette).
