We’re united by a shared commitment to excellent and equitable health care.Position SummaryThe Social Worker (Spiritual Care/Education) is responsible for the delivery of social work services to all Residents as well as their families and/or representatives. In addition, the Social Worker will provide spiritual support to residents and families and/or ensure that access to spiritual care is made available to all residents and families. Ensures related legal and other requirements are implemented and followed, including Manitoba Health Standards for Personal Care Homes. Works within Golden Links Lodge’s Mission, Vision & Values, and in accordance with its policies and procedures.QUALIFICATIONS

  • Registration / Education:
  • Current registration Manitoba College of Social Workers required
  • Baccalaureate degree in Social Work from a recognized and accredited university required
  • Experience
  • Minimum of two years of social work experience in a long term care setting required.
  • Demonstrated computer proficiency with Microsoft Office (Word, Excel) and Outlook required.

REPORTING RELATIONSHIP

  • Reports to the Director of Care (DOC)

HOURS OF WORK

  • Primarily days, flexibility in hours of work is required.

RESPONSIBILITIESI PROFESSIONAL PRACTICE1.1 Applications and Admissions– Assesses appropriateness of applications and admissions, in consult with the DOC or designate as required– Conducts information tours, pre-admission visits and completes assessments for applications and/or consults to Golden Links Lodge (GLL).– Discusses new applications with the DOC or designate.– Manages GLL waiting list and coordinates admissions, transfers and discharges– Coordinates admissions, including communication with applicant / family, Home Care, Long Term Care Access Centre and GLL staff.– Ensures pre-admission packages are provided and completed prior to placement.– Reviews financial forms with Business Office to ensure accurate completion prior to acceptance for admission.– Maintains facility brochures, resident information handbook and pre-admission package. Coordinates regular reviews and ensures content is current.

  • Introduces Residents and family to GLL
  • Monitors and assists with the Resident’s transition to GLL and/or to long term care
  • Introductions to GLL staff.
  • Inputs admission data into MDS (Resident computerized assessment)
  • Provides psycho-social assessment and support. Follows up as required with Residents, families and GLL staff.
  • Works with Nursing staff to ensure Resident needs are communicated and met.
  • Care Planning
  • Schedules, chairs and compiles reports for initial and annual interdisciplinary care conferences, and follows up as required.
  • Completes initial and annual social work assessment for care conference
  • Completes Geriatric Depression Scale and Mini Mental as needed or requested.
  • Communicates any concerns regarding care planning or conferences to the DOC.
  • Utilizes MDS Care Plan during Care Conferences.
  • Facilitates Resident / Family Council
  • Schedules meetings & guests. Posts and distributes minutes.
  • Reports any recommendations /concerns to the appropriate department for follow up and reports back to Resident Council on findings, and documents discussion of same in minutes.
  • Informs DOC and CEO of any issues or concerns, including follow-up.
  • Circulates minutes to DOC and CEO.
  • Ensures annual review of Resident Council Terms of Reference by Resident Council. Forwards recommendations to DOC and CEO.

1.5 Other

  • Ensures annual review of Residents’ Bill of Rights
  • Provides palliative support to Residents and families as appropriate.
  • Circulates sympathy cards for GLL staff to sign for all deceased Resident families
  • Coordinates ethics consultations as requested, in consultation with the DOC.
  • Coordinates completion of Advance Care Plans or Health Care Directives
  • Participates in Family Information Nights, arrange guest speakers/educational component as requested by DOC.
  • Contributes to Golden Links Lodge newsletter
  • Provides recommendations for updates / changes to Social Work departmental policies. Participates in other policy reviews as requested.

II CONSULTATION2.1 Provides internal & external resources and supports as needed by Residents and/or families.2.2 Acts a liaison with the community; responds to inquiries regarding GLL admissions.2.3 Completes annual review of Order of Supervision as required by the Public Trustee2.4 Consults with the Office for Protection for Persons in Care and Geriatric Mental Health Team as requested. Works with the Director of Care and nursing staff to ensure recommendations are implemented in the care plan.2.5 Maintains knowledge of current legislation and legal issues pertaining to Resident care, i.e. Protection for Persons in Care Office, Health Care Directives, Advance Care Plans, Power of Attorney, Public Trustee, Committee, etc.2.6 Completes forms for consults, referrals to special care units or transfers, and assists nursing as requested.III EDUCATION SUPPORT

  • Demonstrates professional responsibility and accountability for own ongoing professional education.
  • Delivers or arranges Resident, Family and Staff education as requested.
  • Work in conjunction with Human Resources to develop, implement and track mandatory education for all staff of the facility.
  • Deliver and participate in education modules while adhering to Manitoba Health Standards.

IV QUALITY INITIATIVES & MONITORING

  • Supports quality initiatives and submits quarterly reports including Bill of Rights Audits, summaries social work activities and care conference concerns.
  • Coordinates & compiles initial and annual Resident satisfaction surveys
  • Maintains data on wait list, bed utilization, admissions and deaths on facility statistics spreadsheet. Completes WRHA monthly bed utilization report.
  • Completes other reports as requested by the Director of Care or CEO and/or designate.

V LEADERSHIP

  • Participates in Accreditation & Standards preparation.
  • Develops and maintains departmental policies and procedures.
  • Participates in fundraising initiatives to enhance Residents’ quality of life
  • Other responsibilities as requested

V SPIRITUAL CARE SUPPORT5.1 Responsible for offering spiritual and religious accompaniment adapted to the needs of all residents.5.2 Responsible for the planning, presentation, and evaluation of spiritual and religious programming.5.3 Ability to influence, inspire, guide the development of spirituality in the workplace.5. 4 Work collaboratively with the Recreation Program on spiritual support and care for all residents.5.5 Provide spiritual access for all residents and families that request.This term position may end earlier.This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment.
