St.Amant is guided by its core values of Collaboration, Hospitality, Excellence and Respect.Position Summary and Scope of Service:The Registered Nurse/Registered Psychiatric Nurse provides direct nursing services to individuals with physical and intellectual disabilities – with a focus on health promotion, restoration, and maintenance, prevention of illness and disability and palliative care. The RN/RPN is responsible for providing total patient care but may delegate some aspects of care to other health care team members in a staff mix model – e.g. Resident Assistants. In addition to providing direct nursing services, the RN/RPN functions as a key member of the professional health care team and is responsible to provide day-to-day instruction and feedback to the Resident Assistants assigned to their care team.The Registered Nurse/Registered Psychiatric Nurse reports directly to the Resident Care Coordinator of the living unit(s) to which they are assigned. The RN/RPN is accountable for their own practice and is also accountable to the Coordinator of Health Services to demonstrate clinical competency and effectiveness in keeping with the provisions and requirements of the Registered Nurses Act, Registered Psychiatric Nurses Act, College of Registered Nurses of Manitoba (CRNM), College of Registered Psychiatric Nurses of Manitoba (CRPNM) and in accordance with the policies and procedures of Nursing Services within Health & Transition Services.QUALIFICATIONS:Education:

  • Graduate of an approved RN or RPN educational program (Diploma or Bachelor of Nursing)

Professional:

  • Holds current active practicing membership with the CRNM or CRPNM
  • Responsible to maintain competency in CPR/Code Blue

Complex Respiratory Care Service positions require a minimum 2 years prior acute care experience, preferably involving respiratory care
Practices within the professional, ethical, legal and legislative parameters of the CRNM or CRPNM Standards of Practice and Canadian Nurses Association Code of Ethics * Adheres to PHIA guidelines of confidentiality of residents/family health information and respects the confidentiality of personal staff information

  • Assumes responsibility for remaining current in their area of professional nursing practice and seeks opportunity for ongoing professional growth and learning
  • Recognizes limitations of own abilities and seeks direction and consultation as necessary and accepts accountability for own actions and decisions
  • Supports and participates in research related to clients and the development of evidence based nursing practice
  • Models professional behavior with residents, families and other members of the care team
  • Ability to make effective use of computer technology necessary for effective care planning, documentation and communication (e.g. MDS Software, Microsoft Office software)

Personal:

  • Demonstrates ability to function effectively as a team leader and team member and possess the emotional stability necessary to serve as a model regarding cooperative teamwork, conflict resolution and quality improvement
  • Demonstrates strong leadership and organization skills, including the principles of delegation and supervision of care delivery by other members of the health care team (i.e. Resident Assistants)
  • Demonstrates effective written and verbal communication skills
  • Demonstrates knowledge of and support for the Mission and Philosophy of St.Amant and the goals and objectives of Health & Transition Services.

DUTIES & RESPONSIBILITESClient Care Management:RN/RPN’s provide comprehensive direct nursing services to assigned clients, in keeping with the scope of Nursing Service’s mandate and the full scope of clinical practice identified by the CRNM, in the following areas:Assessment Duties include:

  • Assess clients using a variety of resources and focusing on biological, emotional, socio-cultural, spiritual and quality of life factors
  • Use knowledge from registered nursing/registered psychiatric nursing and data from other members of the health team to analyze and interpret the data necessary to identify potential and actual health needs/problems
  • Perform ongoing assessments to identify changes in client health status

Planning Duties include:

  • Develop individualized plan of client care in collaboration with other health team members
  • Include the resident and their family, as possible, in developing the individual, focused nursing/health plan of care
  • Support individual health and growth and development by building on personal, family and community strengths and established support systems
  • Supervise the implementation of the individualized plan of health and personal care for assigned clients

Treatment Duties include:

  • Organize work for the efficient delivery of client care
  • Provide direct nursing care to an assigned group of clients
  • Provide health information to clients, families and legal decision makers in an appropriate and timely manner
  • Facilitate a least restrictive living environment and promotes an awareness of clients rights and a client focused approach to care
  • Delegate care appropriately based on assessment of client needs and competence/roles of care providers and monitors outcomes of delegated care
  • Communicate health care concerns to the physician and coordinates residents to be seen at the regularly scheduled medical rounds
  • Demonstrate competence in the performance of technical skills, e.g. administration of medications, treatments and use appropriate technology to provide safe, effective nursing care
  • Promote and support a living and learning environment that focuses on the individual’s abilities and promote physical, mental, social, emotional and spiritual health
  • Work with all members of the health care team in implementing the individual resident care plan

Evaluation Duties include:

  • Objectively evaluate the effectiveness of nursing interventions by comparing actual outcomes to anticipated outcomes
  • Participate in coordinating the appropriate links with physicians, nurses and other health care disciplines at Health & Transition Services to ensure continuity of care
  • Work with clients, families, legal decision makers and other members of the health care team to reevaluate and modify interventions according to changes in health status and assessment findings
  • Advise the Resident Care Coordinator of any significant changes in resident health status
  • Maintain complete and accurate documentation of all relevant healthcare concerns, observations and interventions in the health record, using the approved method of documentation
